Follow the steps below to request your first appointment at Generation Fertility.
It's a simple three-step process and we'll guide you through each one.

How It Works

1. Submit the form below

Tell us a little about yourself and what brings you to Generation Fertility. If your partner will be part of your care, you can include their details too.

2. Get a physician referral

A referral from a licensed physician is required to see a fertility specialist. If you have a family doctor, they can fax it directly to your preferred Generation Fertility location. No family doctor? You can get a same-day referral through our telehealth partner, Tia Health.

3. We'll be in touch

Once we receive your referral, our team will reach out to book your consultation and give you access to your secure patient portal.

For inquiries unrelated to booking a new patient appointment, we invite you to submit a general inquiry.
